Copy this into an email and replace the [bracketed] parts. Ask for specific values and evidence for one
named product, component or batch, with a due date and a single point of contact.

Please provide, by **[due date]**:

1. **Material composition** — main materials and their shares (% by weight).
2. **Recycled content** — share of recycled material, with the basis of the figure.
3. **Substances of concern** — whether any REACH SVHC is present above 0.1% w/w; if yes, the substance
   name, CAS number and the SCIP dossier number.
4. **Country of origin / place of manufacture** for the supplied item.
5. **Supporting evidence** — test reports, declarations of conformity or certificates (PDF), each dated
   and referencing this product/component.
6. **Point of contact** for follow-up questions.

Please send values in writing rather than as a phone confirmation, and tell us clearly if a value is
**not known** rather than estimating — we record gaps openly and follow up.
